On the following web site you will find the questionnaire as an online form. It is comfortable to fill in, the single questions can be answered just by clicking on the different answer boxes. 5 questions should be completed by entering a word or a phrase inside the empty space. Answering all questions will take you approximately 20 minutes. After you have completed all questions please activate the button "submit" to send the questionnaire to me.
